Rhode Island’s failure-to-file penalty is 5% of unpaid tax per month, capped at 25%. The failure-to-pay penalty is 0.5% per month, also capped at 25%. Both can run simultaneously on an unpaid balance. Filing Form RI-4868 and paying in full by April 15 stops both from accruing.
Rhode Island’s two-condition approach is similar to other states that require both federal approval and a zero balance for the automatic extension to apply. If you owe, RI-4868 plus full payment by April 15 is the path – then file your return by October 15, 2026.
